Flexible Hours, Outpatient Opportunity in New Haven County

Location: New Haven County. Close to everything, easy drive to NYC and Boston. 33 miles southwest of Hartford and 77 miles northeast of New York City. Facility: FQHC serving communities throughout Connecticut, with more than 20 locations. Opportunity: Opening for a Family Nurse Practitioner. Provides primary care services in a federally qualified health center that provides primary, dental and behavioral health care services. Appointments are schedule as well as walk-ins. Target is 15 patients per 8 hour day for an experienced APRN. Behavioral health patients being seen at the clinic will be offered primary care services while waiting for intake. Other staff include: VP Health Center Operations, Medical Director, Medical Assistants, Pediatrician, RNs, front desk staff, care management and behavioral health staff. Initially adults only, anticipate serving lifespan early next year Licensed RN and Licensed APRN in the State of CT. Active DEA controlled substance registration and an active CT controlled substance license. Registered Nursing degree and a relevant Master’s degree required. Must be Board Certified. 2 years primary care experience required. 4-5 years experience preferred. Must have MAT experience and already suboxone certified. Ideally we are looking for someone with a waiver for up to 100 patients. If they’re currently at 30 and close to being able to go up we may be able to consider them.
